Why AI value is realized where clinicians work—inside existing systems, decisions, and accountability.

The thesis

AI does not create value in isolation. Value shows up when capability is integrated into day-to-day practice—where decisions are made, documented, audited, and acted on.

What breaks in reality

  • Sidecar tools die in the last mile: extra clicks, extra context switching, unclear accountability.
  • Outputs without ownership create risk: “who signs off?” becomes the actual question.
  • No integration = no learning loop: you can’t measure impact or improve reliably.

Principles

  • Integrate at the point of use (where the user already is).
  • Design for accountability (who reviews, who approves, who escalates).
  • Treat monitoring and telemetry as part of the product.

What I look for

  • Clear “before/after” in how work happens (not just how text is generated).
  • Evidence that users trust the system enough to use it under time pressure.
  • KPIs tied to clinical and operational outcomes, not vanity metrics.
